He'res the sample of visa.
Reminder:
I've inquire some travel agency about processing of tourist visa, they require me to buy an airline ticket and book a hotel from them and they have expensive service fee.
( Visa fee: 1,400 plus Service fee: 1,200 = 2,600 php ) and not only this, flight Itinerary also cost
USD $ 25 plus hotel reservation an airline ticket. So, this quite expensive for me and I am glad that I found a travel agency which process an individual tourist visa and no other charges other than service fee and visa fee.
You can apply your tourist visa 3 weeks prior your intended travel date.
